Here are some simple planning documents, plus our
contract and a link to our payment center:

- Contract Form (submit this electronically, or print
and send to us)
- Wedding Planning Guide for special dances
- Wedding Introductions and common special dance songs
- Deposit Payment by Visa or Mastercard

We accept Visa and Mastercard, Checks or Cash as a form of payment. Deposit payments are required to be officially booked with us. Deposit checks are made payable to Mid-West Family Broadcasting. Remaining amounts are made payable to the personality performing at your event.
